This project seeks to achieve better communication within the nursing home facility (Waxman, Roussel, Herrin-Griffith & D’Alfonso, 2017). This improvement applies both to the patients and the clinical professionals. With better communication, the personnel and the patients will have better relationships, consequently improving the care processes.
