History and Physical Exam

If you are in a clinic, jail, or in a situation where you do the history and physical exams as a registered nurse, you will need to know how to interview a patient for a health history and doing a genogram.

Attached is a document with the criteria for writing your complete health history paper. Also attached is a sample paper turned in by a Stratford U. student who received an A on her paper.

You will be interviewing another student who is acting and pretending to have a particular type of illness and the student will fabricate a story of his/her illness so you can do a thorough health history on the student.

There are sample health histories attached on clients with a CVA and an MI and one on cirrhosis.

Gary has been married to his wife for seven years. The marriage has had its ups and downs, but it is intact and relatively happy. Gary’s wife is a successful business owner. The economic upturn affected his family, adversely affecting their savings, investment and preventing them from living the life they were accustomed to. Gary has a master’s degree from Johns Hopkins University. After getting his MBA, Gary started his business. He has one full-time employee and a receptionist/data entry person who works hourly. Even though he has a large customer base, the clients tend to pay later than he wishes, mostly sixty days or more. As such, Gary has to, more often than not, request his virtual accounting office ask clients for payment, an aspect that has negatively affected his cash flow. Gary is going through a tough time emotionally and financially. The cash flow issues affecting his business are making it hard for him to meet his financial obligations to his employees and family.
